Name Change and Personal Information Update
Name Change Form
Students are required to use their legal name in NU’s data systems. Transcript, diplomas, and enrollment verification will reflect the legal name of students and alumni.
To update your legal name or to update your personal information in NU’s records you must submit the Name Change Form along with one of the following legal documents:
- Court order document
- Driver’s license
- Passport
- Social Security card
